In order to participate in the services provided by Pawtherapy K9 Aquatic & Massage Centre, we

require the forms listed below, to be completed. Forms must be emailed to (info@pawtherapy.ca)

or dropped off to our facility, at least 24 hours prior to your scheduled appointment.
